Account Transfer Form
Is needed to transfer cash or securities from one institution to another; please be advised that only like accounts can be transfer to like accounts (joint account to another joint account), and that the most recent statement of the account must accompany the form itself.

World Cup Suitability Form
is needed to qualify individuals competing in the World Cup Championship of Stock Trading®.

Corporate Account Authorization
authorizes the opening of an account in the name of a corporation.

LLC Account Authorization
authorizes the opening of an account in the name of a LLC.

Joint Account Agreement
allows the opening of an account in the name of more than one individual.

IRA Application
allows for the opening of a self-directed IRA account.

Roth IRA Application
allows for the opening of a self-directed Roth IRA account.

Partnership Account Agreement
grants authority for members of a partnership to purchase and sell securities for the account.

Limited Partnership Account Agreement
grants authority for members of a limited partnership to purchase and sell securities for the account.

Letter of Direction
allows a representative to follow a trading system in your account.

Limited Trading Authorization
gives the ability to trade in any and all securities to a party other than the account holder, limited to purchases and sales only.

Trustee Certification
gives trading authority to named trustees.

Certificate of Foreign Status
is required for non-U.S. citizens.

Disclaimer for Option Spreads
is required those trading option spreads in retirement accounts.

Funds Transfer Form
enables the transfer of funds from your brokerage account to your AMA account to cover checks written.

allows customer to draw funds via check from their trading account.
